- UNT/CT Certificate
- Medical certificate form 075-U with fluorography
- Health passport and vaccination certificate 063
- Photos 3x4 (6 pcs.)
- ID card (3 copies)
- Parent's ID (for minors)
- Certificates from drug, mental health, TB dispensaries
- Criminal record certificate
- Military registration certificate (for men, copy)
Master's Degree
- Higher education document (original)
- Copy of ID document
- Copy of marriage/divorce/name change certificate (if diploma issued in different name)
- Four photos 3x4
- Medical certificate form F-075/U
- Copy of work record book or employment certificate (if available)
